Below are our example forms. Once you have a confirmed rental/booking, you will be required to fill out these forms. For your convenience, wait for them to come from BonVoyage, so that no downloading & attaching is needed. We require these forms to be digitally signed.
This is an example form. BonVoyage will send a customized form based on your unique needs and rental experience after a booking request has been received & confirmed.
This is a document formally requests proof of insurance from the renter’s insurance provider. This form is required for every towing rental request made in-house with BonVoyage.
This form is used to obtain permission from the credit card holder to charge their card for specific goods or services. This form is mandatory for all credit cards used with BonVoyage.
